   FYI, I’m not a Costco member and didn’t input a number in that field. It didn’t have an asterisk, so that means you don’t have to fill it out.

    Just got mine and I’m not a member of costco. Random numbers work, if they wanted it to be member only they shouldn’t allow it to work.lol. I love free samples!

    it’s much faster to fill out these forms if you don’t bother to fill out the ‘questionnaire’. most times it is not required.
