2024 Update: I originally posted this two years ago in February 2022, but I just tested the request form with my family's address and it worked just fine. So I believe this freebie is still active for anybody who originally missed out!
Just a note: It doesn’t appear that you can use this unless you’re on Facebook … by the time you get done filling out the form, the box at the bottom requiring you to login to FB or join it covers up whatever you would need to click on to “send” your completed form.
If you end up on the Canada or US website, and need to get to the other one, use the dropdown in the upper right.
