It does appear as if this is meant for bulk buyers of student planners (like school officials), but Positive Promotions has held offers like this going back to (checks HIF archives) 2007! So they have a fairly good track record of fulfilling freebies like this, but they just tend to expire quickly. I'd be surprised if this form was still active tomorrow.
