   After filling out the form, it didn’t go to another screen. I re-did it to make sure I didn’t hit the “reset” button and it still did the same thing. Anyone else having this problem?

   to #4, after you filled it out and pressed submit, did you get a thank you msg at the bottom of the info section? thats what happened to me, but it appeared to go through.

   Same thing for me, but beneath the form it said Thank you, so I assume it went through.

   I entered my info twice because it appeared not to go through.

   The second time I entered it said “you can only request one sample”.
   So I assume it worked.

   I tried twice and both times received a message that said”failed to submit”. Oh well, maybe I’ll try again later perhaps the site’s ‘not in the mood.’

    aphrodite…i had the same problem, went thru this page and it went thru…btw i did have to resubmit in order for it to work

    I filled out the form, and when it didn’t “take”, I realized I forget to check off my gender. Duh!

    It didn’t work for me either. Kept saying failed to submit.

    I had to reload the page for the form to come up. When I clicked “Submit” – the info I’d entered disappeared, and the text “Thank you.” appeared very small beneath the form.

    By the way – why is “2008″ an option for the date of birth? Are one-year-olds requesting this??

    Hey, I just recieved my tube, and I gotta say, this is the worst tasting jelly I’ve ever had! Doesn’t even spread that well on your toast! Think I’ll go back to Smuckers.
